How to provide feedback:
Monash Youth and Family Services (MYFS) is committed to providing the highest possible professional standards to service delivery to young people and families that live, work or play in the City of Monash. In order to maintain a high standard of service delivery, feedback on your experience is highly valued.

Complaints and suggestions
Receiving positive feedback about the services MYFS provide is very important; it lets us know when we are doing things well. Therefore, if you are happy with the service provided we would like to encourage you to let us know, so we can maintain a high quality if service delivery. Also, if you have a suggestion on how something can be done better or you have any other ideas, please fill out the adjacent feedback form.
Complaints Procedure
If you feel the service provided does not meet your expectations, or you have concerns about any part of MYFS service provision, you have the right to complain. All complaints will be taken seriously and will be followed up. HAVING ISSUES WITH A MYFS PROGRAM, GROUP OR FACILTY? Let your worker or program worker know if you have issues with the building, programs or program content or program group. They can help follow up any issues.
Having issues with a worker?
If you feel uncomfortable, MYFS encourages you to speak directly to the worker you have concerns with to attempt to resolve any issues. If you are not satisfied with the response, or you do not feel comfortable doing so, you may ask to speak to the team leader or service coordinator. Alternatively, you may fill out the adjacent feedback form to highlight any of your concerns.
